Fill out and fax this form to us to have a worthless security removed from your account.
Limited Trading Authorization
This form is needed if someone other than those listed on the account will trade for you.
W8Ben-Form
This form is needed for non-US residents Certificate of Beneficiary.

W9Ben-Form
 This form is needed if you opened an account and need to correct taxpayer I.D. information.

ACAT Transfer
For transferring balances and positions.
 Irrevocable Stock or Bond Power
Used to deposit stock certificates into account.
Option Agreement
This form is needed if you wish to trade options.
Margin Agreement
This form is needed if you wish to trade on margin.
Trustee Certification of Investment Powers
If you would like to open a Trust Account, you need to complete and sign this form.
Transfer on Death
Designation of Transfer on Death Beneficiary.
Sole Ownership Letter
If you would like to open a Sole Ownership Account, you need to complete, sign, and mail-in this form.
IRA Distribution Form
This form is needed to request a distribution from your IRA.
Letter of Authorization (LOA)
This form is needed if you want to transfer funds or securities from an account under one name to an account under a different name.
LLC Authorization
This form is needed if you want to set up an LLC Corporation account. Please also download the Corporate account paperwork.

Click on the type of account you will be opening and the necessary forms will be downloaded to Acrobat Reader for printing. You must complete all appropriate forms in full before mailing them to GlobalTradz, Inc.
